How do bans work

is it based on the severity of the action or a sliding scale?

They work with both.

e.g. if you ninja in a dungeon for the first time: 5 days ban. If you do it in a raid: 60 days.
If you do it a 2nd time you'll get banned for longer. Iirc it's doubled at 2nd infraction and may lead into a permanent ban at some point.
