How do I remove the appearance of something that I have used a scroll on?
Is there a specific scroll that you use to remain appearances?

There is, however the only way to get it is with coins (5) - if you really dont like the look of an item, just overwrite it with its original look, or with something else by another Scroll of Deception
You can only return it to it's original look using scroll of purification, otherwise you get an error sounding like "That item is already transmogrified". You can however:
Use a different transmog on it without purification
You can still use that item's original model for another item (let's say I transmogged by pve back to smt else, if I use a scroll of deception on the transmogged back, I get the original essence which I can put on let's say a pvp cloak)
Visual bug. It only appears you don't have the bonus when you hover on the item but in reality you do - it works just fine.
