Unique-Equipped Gems?

Yo,

So I was randomly inspecting people and there I ran into a peculiar case of the following character named Neriak from the guild Play for Logs.

As you can see, Neriak has a 10attack power 6 stamina gem in his helmet, but also in his chest piece. As far as I know, those gems are Unique-equipped gems from heroic dungeons. Could someone tell me how this is possible? Am I missing something?

One is from Nightbane (Quest), other from Heroics.
