Seal of Vengeance working as intended?

Its supposed to add a debuff every hit, but it rarely even adds the debuff, stacking 5 is near impossible? is this a known issue or is there something im not aware of.

It adds the debuff every time you melee, not off of special abilities. So it will be every ~3 seconds depending on what speed weapon and how much haste you have.

This is likely the reason for what you were experiencing. However, the issue was resolved on the day it happened.
