Well, I tried it myself and we got it in the first go (guild run).

That's not what I meant.
It means that the instance / quest is definitely do able and is 'not' bugged for everyone. Which also means that if you have failed in 1st go, maybe you'll get it in 2nd try.

Quest not being completed for EVERYONE is different then being completed for some and not being completed for others. Right?

WRONG! I just did OHF. Thrall bugged as ****. He didn't despawn, after we freed him, he charged to all the waypoints very fast. And we found ourselves with 10-12 mobs because the scripts don't work properly. We managed to overcome this, hard btw, and then second and most important bug is when we killed the last boss NOONE got their quest completed.
Its very, very frustrating.
