this happens when the server is rashing or you are using alt+f4 in fights (and you log-in)

No, this has been here since the relog/res bug, your body will still be there even though you died before logging out or closing the game.
The most rare and unknown bug is the zombie glitch whereas you can still move and fight back with 0%, but you take negative damage. So far this has happen about 6 times in a span of 3 years.
