Ahn'kahet: The Old Kingdom Insanity bugged

Herald Volazj's Insanity spell is definitely bugged, and this has happened to me before. I don't know what is causing it, but my teammates and I are killing the copies but not being transferred afterwords out of insanity or even to a teammates insanity. We're all just stuck watching Herald Volazj doing his casting animation and the fight becomes soft locked. Is anyone else experiencing this, if so do you know a cause or fix?

I don't know why it's happening, but the only way to fix it right now is for everyone to get out of the dungeon (there's an exit behind the boss where you can run out), then the boss will reset and you can try again. It has always worked on the second try for the groups I've been in!
