Who is known to have it's limitations and default filters.
First off, it's limited to 50 characters at most.
Second, by default, it shows players around your level and in your zone.
/who isn't a population statistic and can't be trusted really. If you do a /who for each level and then multiple ones for levels which have 50 players then you could get all of the players listed.
There's addons like CensusPlus which do a better job of analyzing the population. Ofc, the addon will show you people's alts, but so will /who if you don't know them by names.
/who doesnt include players in character creation screen

/who has cooldown, u cannot spam it
keep in mind some instances (pve & pvp) are on separate servers and /who only includes characters on the same server (not realm, but server - actual hardware machine)

That should show you all players online on your faction. Usually the pop on Frostwolf or Outland is so low that it doesn't exceed the 50 players limit of /who so it's possible to count players that way.
