An encounter is in progress

We just got that error message "an encounter is in progress" on Ulduar25 and we couldn't come to instance for 15 minutes. XT-002 disapered several times after pull, razorscale haven't been pulled but we got message "Harpoon is ready". So we decided to reset instance. No one were inside and we couldn't come it. Who faced same problem before? How to fix this bug?

We had something similar happening to us. Abomination, the off tank kept getting dispelled. Well it turned out that one of the pugs just wanted to troll PP cuz he didn't like the fight, so you can never be sure who went to lets say razor and sstarted the event. I even had rouges with speed boots run around in hcs and pull all mobs and then act like the rest of us - WTF IS THIS, how it go there and etc.
