Failing through the ground

Im having some difficulties with as u know it failing through the ground. It happened in arathi highlands 3x times now. Any tips?

Is this happening everywhere in that area or just in a certain spot?

If this is Cata/MOP this usually happens when using a spell/action nearby small hills and or terrain. In some situations, enemies either send you going down under, or shoot you into the sky like a rocket.

I'm taking a guess at OP situation, worse case scenario, fake floors.
