Is donating 10 dollars once enough to skip the queue forever or do you have do it every x amount of months/weeks? Referring to Icecrown if that makes any difference

It is. Once you donate, your ACCOUNT becomes PREMIUM. Premium status isn’t related to the server.
It might be slightly different for Lordaeron, but it has no queue anyways.
