You do bgs/raids, you get transmog scrolls (low drop rate), 1 scroll = 1 item, you need to have the item you want to take the appearance from in your bag.

here is a video showing exactly how its done. you use the transmog scroll on the item you want to "save" the appearence of and then use the essence on the item you want to change.
