If you gave it away, ask the person you gave it to.
If you lost a roll, ask the winner to give it to you. (Assumption: you were a tank, item winner was a DPS or a healer)
If you sold it to vendor, check the buyback.
If a DPS needed it and won the roll, you can make ninja report. To make a valid report, you must provide screenshot(s) of the incident. Otherwise it's no use.
