How does transmog work?

The original post said that you can 't use green or white items for a transmog. Has that changed?

Make sure that green item is soulbound to you before you try to extract appearence

Do I have to do that with every item?

Also, can a plate head for example only be transmoged with a plate head or I could use a cloth or a leather?

plate only goes to plate, same with all items, and class restricted items like tier items only works for that class.

Acherus Knight's Hood(well,the whole dk starting set) isn't class restricted technically(just like T3 gear,which has no class restriction),you can buy it for coins from the shop.

Nope, the mog only gets removed.
