The bug fixes and reworks that will be ported over to Cataclysm from the realm "lordaeron"... does this mean a new x1 realm will be made or just the fixes are being put on neltharion?

It will be put on Neltharion. Cataclysm doesn't have the population for another realm anymore

it only means we'll be getting content from Lordaeron that's word-for-word identical, meaning some maps that have been revamped by the Cataclysm patch will not have appropriate instances ported, nor would the quests for those zones, but (probably) everything in Outland and Northrend + Vanilla raids, some dungeons and quests, should all be eventually ported over to Neltharion.
The core itself is unlikely to change unless (unlikely) Warmane decides to try and make a cata version of Lordaeron... which probably wouldn't be overly populated and thus wouldn't be a good way to spend their time
