Burning Crusade Unable to pick up quests as part of a chain

i have run into countless examples of being unable to obtain a certain part in a chain, preventing me from being able to continue with it. The two most notable examples I can thing of are my inability to gain "Against the Legion" from Altruis the Sufferer as part of "The Book of Fel Names" quest chain and my inability to gain "Attack on Manaforge Coruu" from Exarch Orelis as part of the "Deathblow to the Legion" quest chain (yes I had completed the parallel quest chain to shut down Manaforge B'naar). I completely understand the lack of polish on Vanilla quests, they are just there to get us levels. But on Outlands stuff? That's what we are here for, and in many cases those quests provide us with our pre-raid BiS gear, and are very important. Please try to resolve some of this dangling quest chains, thank you!
