There can be multiple mobs up at the same time. They just spawn after some time not when another dies. I have both of those achievements. All of them should have about same respawn time so there shouldn't be more or less uncommon rares. It depends on luck. Only Vyragosa has a good reason to be more uncommon.
Can confirm every single Rare in Outland, they spawn yes, but some will/might take WEEKS a part because of the amount of times the server crashes/gets restarted. Nuramoc without a doubt being the rarest thing probably in this game to see spawn as of late, it will be near impossible with the amount of people camping his spawns. I feel that molten, due to the amount of server DC's, should seriously consider upping the spawn timers, or making his spawn more available.
Does NPCscan 3.3.5.5 addon works on WotLK realms, because yesterday i did a full walk over the spawn areas in all of Northrend and did not find a single rare animal
Npc scan does works. You just have to learn to clear the cache every time you get in get to be sure it will pop up when it found something. Now, about Nuramoc, it spawns pretty nice. Have it on a twink and an 80 hunt. 2 buddys have it also and we also saw it killed few times. The thing is that afk'ing in Dalaran won't make this pet jump by itself in your colection. Its 3 spwn points have to be surveilled all the time or a nifty huntard might get it.
Also i wish u good luck with the northrend ones. King krush and Loque will be a pain.

Where do you look the spawn points from, i have npcscan overlay addon which marks areas of the map with colors for different bosses but so far i couldn't find any boss in those areas so they might be wrong for warmane

Npcscan overlay is mostly correct here but some mobs stay in their spawn point instead of moving around. You can find exact spawn points for Warmane on Openwow.
