"Hateful Strike is determined by three requirements:
Proximity: Targets must be within melee range of Patchwerk.
Threat: Targets must be less than first in threat, but at least second/third highest threat. This requirement does not apply if only one target is in melee range.

Each requirement is dependent on the ones before it, so a high-threat player isn't considered if she's out of range. Similarly, a high-health player isn't considered if he's low on threat."
But i hear random people in raid asking people to go skinnydipping in green slime, remove stamina buffs and what not?
So what is the true mechanic?
Melee dps will get hatefulls even if they are not 2nd/3rd on threat, that is they try to lower their hp in hopes of that they will be lower than the tanks even after the tanks has been hit. That being said, if the hatefull tank healers are on point, there is no need for melee to take such precautions.
But that is not how it worked in wotlk. The green skinnydipping worked for the Orignal Naxx and not for the wotlk naxx. Which means that the encounter is bugged? because even when mdps dint out threat the OT, he still took a hateful. Is it really intended to work like that??
Honestly, I can't remember exactly how it was during retail. It's a few years ago, and I wasn't playing a tank then so I not sure of the specifics of hatesfulls during retail. What I said in the last post is how it's here though. If it's as you said, not correct, I have no idea if it's intentional or a bug. I guess a dev would have to chime in for you to get an answer to that.
There are 3 criteria of the spell:
1. He will only target players in his melee range
2. He will select the 3 players under the tank by threat
3. He will select the one of the 3 with the highest hp
He first selects the top 3 by aggro and then selects the highest of those (excluding the current tank obv)
