/cast vanish

simple 'oh ****' macro. cleanse dots, and vanish. all three of these abilities are off the gcd, and when i am out of combat testing it, hitting this macro works perfectly every time. however, when in combat and i need it to actually work, it very often fails to vanish, sometimes even fails to cleanse. wtf is going on? and what do i need to do differently to get this to work reliably?
