Items in dungeons not up for rolls

Today I did FoS HC and NES dropped and I didn't get to roll on it. In fact, nobody seemingly did, and one of the party members took it without rolling for it. That's not a problem for me since I already have it, but I have to wonder, does this mean that nobody was eligible to roll for it since we had it already? Or maybe I didn't see the rolls because I'm not eligible myself but they rolled on it? Is this something new? Does this apply to all items or only the most sought-after ones?

I remember a similar experience years ago with battered hilts. Once the quest was started I couldn't roll on them anymore. But this could be something else because you said someone looted it and that's a unique item, not even unique equip. I guess they could have deleted theirs and picked up the new one.

Good call, it hadn't even occurred to me that the item is unique. That makes sense, thanks.

It's possible that they did roll for it, but I didn't see the rolls on my chat, just someone picking the item up.
