For endgame gear you need Coins (you get coins by donating)

No, you can buy those only with coins. With points you can get up to item level 239. You can check the items and their prices on Store page.

Achievements from Frostmourne and Blackrock don't count.
They need to be unique - having 2 level 80 achievements only counts once.
Once you're able to collect points you'll see a "Collect points" next to the amount of points you have here: https://www.warmane.com/account
