It drops from Lady Vashj in Serpentshrine Cavern.

AtlasLoot claims a drop chance of 14%, wowhead claims 6% (although only based on 17 counts).

I've killed lady vashj 18 times now and it hasn't dropped. I'm no stranger to how RNG works, I'm just merely wondering if it could be bugged or have a lower drop chance than I'm expecting. The other items I feel like I've seen drop multiple times.

Nevertheless I will continue grinding it, but in the meanwhile I thought to check if anyone has actually seen this drop, and if so on what realm? I'm on Lordaeron.
