From reading the combat log, it looks like the first Vanish was broken by an Ambush and the second one by a Fan of Knives.

How can a player use Ambush on a target when they are in full invisibility? lol what

Vanish is not "full invisibility", it's an improved state of stealth. Other players can theoretically still see you, especially when they use Shadowstep just before. You had some colosally bad luck there.

Your fan of knives failed to hit anything probably because the other rogue had Evasion on.

My feeling is you get touch by shm aoe the first time, and the second pala buff rekt you.
