Of the three you mention, only on blocks, and only when you have a full 5 points invested in it.

Why does it has to be 5/5 for it to proc on blocks , do u know?

Because it's the 5th point that grants it that capability. If you check the tooltips of the ranks, you'll see it reflected in the tooltip.
