Technical New GM after 90 Days Inactive?

Does Warmane and this expansion (I play Mop) still do Blizzard's 90 day -3 month- inactive rule (I thought it was 30 days until I looked again and saw 90 days)??

Also, is it the first 3 ranks only which can use the Dethrone Feature, which would be GM, rank 1, rank 2? They also need to log in within that week?

For example, in the guild I am in, this would qualify as GM, General, then Major General? Rank 0-2 (1-3).
