Frequent disconnections

Hi, I am playing on Frostmourne and get constantly disconnected from the game. some times it works fine without any disconnections, but at times i get disconnected so frequently . for example take right now within the span of 5 min i have been disconnected 3 times. and before every disconnect there is a huge lag and them boom disconnect.

please help me in identifying the issue if its on my end.

Same happend to me, the server disconnect me always in 30 - 45 secs (Server Icecrown)
